Foa uses an exposure hierarchy for in vivo exposure. For imaginal exposure, she has the person go right to the most troubling memory and describe the different facets of the the experience. I suppose that she does that because if she tried to have them think of a less severe trauma, they would think of the most severe one any way; like trying not to think of pink elephants. She also does not (usually) have them do relaxation exercises during the exposure. IMHO, this is a great idea, as the tacit message of relaxation during exposure is that anxiety is bad. She does have people learn relaxation and grounding fore between sessions.

The book is great for beginners, and has examples of dialogue, much as Barlow has.

Foa recommended that the therapist and person meet "once or twice a week". Which is more frequent than most recommendations. She says that even if the SUDS do not decrease over the 60 minutes imaginal exposure, that overall desensitisation will have recurred: that is a relief to know.

Foa gives an excellent rationale for exposure. It is summarised on page 84 or 85 and given in more detail earlier in the text.
---1. Processing and organising the memory (I supposed this includes dealing with triggered automatic thoughts AND getting what happened as straight in one's mind as possible).
---2. Prompting differentiation between "remembering" the traumatic event and "being traumatised" (This is really good news to tell people).

In the spirit of full disclosure, I am a dog lover and the owner of a Shetland sheepdog that never ceases to amaze me with her intelligence. For most of my life, I have strongly disagreed with the belief, long espoused by the church and philosophers such as Descartes, that animals are soulless and therefore essentially different from humans. It is therefore with great interest that I picked up a copy of 'Chaser: Unlocking the Genius of the Dog Who Knows a Thousand Words', a book that knocks that archaic attitude on its tail.

Its author, John Pilley Ph.D., is a retired professor of psychology at Wofford College who, during his teaching career, taught animal behavior using, as his teaching assistant, his dog, Yasha. With both Yasha and his teaching career behind him, Pilley took to moping around the house until his wife, as any sane wife would, put her foot down and told him in no uncertain terms that he was going to get another dog. Enter Chaser, a border collie puppy that enters the Pilley household and immediately takes over.

Pilley, with his background in animal behavioral research, is not your everyday dog owner and immediately sets out to answer the question, "How much can a dog learn?" Armed with love, patience and an endless supply of free time, he worked with Chaser every day, devising experiments aimed at discovering the limits of what a dog can learn. I use the words work and experiments but the truth is that everything he does with Chaser is play, at least as far as Chaser sees it. Within three years, Chaser had learned the names of 1,022 toys and was able to correctly respond to several commands related to each. She also was able to apply deductive reasoning and retrieve unfamiliar items by process of elimination.

About the Author

Monica McGoldrick, LCSW, PhD (h.c.), is Director of the Multicultural Family Institute in Highland Park, New Jersey, and Associate Professor of Clinical Psychiatry, University of Medicine and Dentistry of New Jersey-Robert Wood Johnson Medical School. Her videotape of clinical work with a multicultural family around issues of loss is one of the most widely respected in the field. Several of her books have become bestselling classics, including Ethnicity and Family Therapy; The Expanded Family Life Cycle; Genograms: Assessment and Intervention; Women in Families; Living Beyond Loss: Death in the Family; and Re-Visioning Family Therapy. About the Author

Ennio Cipani, PhD, a graduate of Florida State University, is a licensed psychologist and a full professor in the Department of Special Education at National University- Fresno. He has published numerous articles, chapters, books, and instructional material (including software) in the areas of child behavior management in homes and schools. Among his latest books are Classroom Management for All Teachers: Evidence-Based Practice, 3rd edition (Prentice Hall, 2007), and Punishment on Trial (Context Press, 2004). He has given many workshops at state and national conventions, as well as continuing education courses for psychologists, social workers, marriage and family therapists, and others, focusing on the effective management of problem child behavior. Dr. Cipani has been doing in-home behavioral consultation for families with problem behaviors since 1982. He has dealt with many families and a variety of behavior problems, conducting assessment and intervention activities in homes and classrooms.

Foa uses an exposure hierarchy for in vivo exposure. For imaginal exposure, she has the person go right to the most troubling memory and describe the different facets of the the experience. I suppose that she does that because if she tried to have them think of a less severe trauma, they would think of the most severe one any way; like trying not to think of pink elephants. She also does not (usually) have them do relaxation exercises during the exposure. IMHO, this is a great idea, as the tacit message of relaxation during exposure is that anxiety is bad. She does have people learn relaxation and grounding fore between sessions.

The book is great for beginners, and has examples of dialogue, much as Barlow has.

Foa recommended that the therapist and person meet "once or twice a week". Which is more frequent than most recommendations. She says that even if the SUDS do not decrease over the 60 minutes imaginal exposure, that overall desensitisation will have recurred: that is a relief to know.

Foa gives an excellent rationale for exposure. It is summarised on page 84 or 85 and given in more detail earlier in the text.
---1. Processing and organising the memory (I supposed this includes dealing with triggered automatic thoughts AND getting what happened as straight in one's mind as possible).
---2. Prompting differentiation between "remembering" the traumatic event and "being traumatised" (This is really good news to tell people).
---3. Promoting habituation.
---4. Promoting differentiation between the traumatic event and similar events (triggers).
---5. Increased mastery and sense of control.
