I primarily work with adolescents with complex trauma and attachment based disorders. When I was first exposed to TF-CBT, I was concerned about its use with complex trauma. Clients who suffer from complex trauma, typically suffer from the developmental effects of the trauma exposure, rendering them quite vulnerable to the day to day stressors several years later when unresolved. With their Stress Response Systems typically compromised, traditional top-down skill building strategies are typically not efficient enough to prepare children with these histories to process their narrative without the possibility of re-tramatization. With TF-CBT being a "16" session based approach (with the caveat that sessions have to be mastered prior to moving on), this didn't seem feasible for the "phase based approach" that is more the standard right now with children and adolescents with complex trauma. The chapter in this book on "Adolescents with Complex Trauma" was such a relief to me. This chapter, for the most part, completely covers what is missing, in my opinion, from the traditional TF-CBT approach when it comes to complex trauma. (You will need the first book to learn about the actual approach - this books talks about the modifications based on different child/adolescent populations and settings).

You will also need to verse yourself in bottom up strategies, and the information that neuroscience is adding to the field of complex trauma. There are many helpful resources at the end of this chapter for further reading. I have found that a good amount of time needs to be spent on teaching the "mapping" of distress episodes, i.e. from environmental trigger, to visceral cueing, to emotional identification, the thought development to behaviors.

It was only after I was in my 50's that I discovered I had ADHD (Attention Deficit Hyperactivity Disorder), a variation of ADD, Attention Deficit Disorder. Oh, I knew I was always doing something, and in my head I could be thinking of three things at the same time, but I could not fully understand why I had to strive so hard to be organized or why it was so easy for me to multitask as a manager/administrator.

Finally, I married someone whose child had been treated for ADD for 15 of her 20 years, and in learning about ADD, so I could be a better stepfather, I read too many books on the subject. I say I read "too many books" because I found myself starting to take a cynical approach towards the author before I was halfway through their book. Most proclaimed that "THEY HAD THE ANSWER" or "THEY GUARANTEED RESULTS," and in some limited instances, I am sure they did have an answer, but I found, as I discovered more children and adults diagnosed with ADD and ADHD, it cannot and, quite frankly, should not, be treated with a "cookie cutter" approach. Otherwise you'll have everyone overmedicated with Ritalin or Adderrall, when maybe neither is needed.

Then, I came across Dr. Daniel Amen's book HEALING ADD, the previous edition (yellow cover), and in it, Dr. Amen gave you a detailed description of how he developed the SPECT immaging process in diagnosing ADD. This new edition, which was published earlier this month, isn't just an updating of a few references, as I have seen when other books come out with a revision.

About the Author

Judith A. Cohen, MD, a board-certified child and adolescent psychiatrist, is Medical Director of the Center for Traumatic Stress in Children and Adolescents, Allegheny General Hospital, Pittsburgh. With Dr. Mannarino, she has received funding to assess and treat traumatized children since 1986 from the National Institute of Mental Health, the Substance Abuse and Mental Health Services Administration, and the U.S. Department of Justice. She is a recipient of the Outstanding Professional Award from the American Professional Society on the Abuse of Children (APSAC) and the Rieger Psychodynamic Psychotherapy Award from the American Academy of Child and Adolescent Psychiatry.
 
Anthony P. Mannarino, PhD, is Director of the Center for Traumatic Stress in Children and Adolescents and Vice Chair, Department of Psychiatry, Allegheny General Hospital. He is also Professor of Psychiatry at the Drexel University College of Medicine. A leader in the child traumatic stress field since the 1980s, Dr. Mannarino has received many honors, including the APSAC Outstanding Article Award for papers published in Child Maltreatment. He is a past president of APSAC and of the Section on Child Maltreatment, Society for Child and Family Policy and Practice (Division 37), American Psychological Association.  

Esther Deblinger, PhD, is Co-Founder and Co-Director of the CARES (Child Abuse Research Education and Service) Institute and Professor of Psychiatry at the School of Osteopathic Medicine, University of Medicine and Dentistry of New Jersey. A longtime collaborator with Drs. Cohen and Mannarino on the development, evaluation, and dissemination of TF-CBT, Dr. Deblinger has published widely on the treatment of child sexual abuse, trauma, and traumatic grief. Her scientific contributions have been recognized with the APSAC Outstanding Research Career Achievement Award and the Rosenberry Award from the Children's Hospital of Denver, among other honors.

I primarily work with adolescents with complex trauma and attachment based disorders. When I was first exposed to TF-CBT, I was concerned about its use with complex trauma. Clients who suffer from complex trauma, typically suffer from the developmental effects of the trauma exposure, rendering them quite vulnerable to the day to day stressors several years later when unresolved. With their Stress Response Systems typically compromised, traditional top-down skill building strategies are typically not efficient enough to prepare children with these histories to process their narrative without the possibility of re-tramatization. With TF-CBT being a "16" session based approach (with the caveat that sessions have to be mastered prior to moving on), this didn't seem feasible for the "phase based approach" that is more the standard right now with children and adolescents with complex trauma. The chapter in this book on "Adolescents with Complex Trauma" was such a relief to me. This chapter, for the most part, completely covers what is missing, in my opinion, from the traditional TF-CBT approach when it comes to complex trauma. (You will need the first book to learn about the actual approach - this books talks about the modifications based on different child/adolescent populations and settings).

You will also need to verse yourself in bottom up strategies, and the information that neuroscience is adding to the field of complex trauma. There are many helpful resources at the end of this chapter for further reading. I have found that a good amount of time needs to be spent on teaching the "mapping" of distress episodes, i.e. from environmental trigger, to visceral cueing, to emotional identification, the thought development to behaviors.

That was one of my thoughts as I read Sherry Turkle's Alone Together: no matter what robots learn to do, they will never learn to write a book as thoughtful, informative, and intense as Alone Together. They would not know how to pose the questions, let alone use such discernment in addressing them.

It is interesting that Turkle chose to discuss robots in the first part of the book and the Internet in the second part. By presenting the "strange" part first, she gives us a sense of how strange our everyday lives actually are, how far we have moved away from enjoying each other's presence.

Turkle quotes children and adults who hesitate to use the phone because it seems awkward and intrusive; it is much easier, they say, to dash off a text or email. At the same time, Turkle points out, because of this very convenience, people expect quick responses. She describes the anxiety of teenagers when they do not get an immediate reply to their text messages. One girl talks about needing her cell phone for "emergencies"; it turns out that what she means by "emergency" is having a feeling without being able to share it.

Turkle shows how our Internet communications mix the deliberate with the unconsidered. On the one hand, people put great effort even into short email messages. On the other, they "test" ideas and expressions in formation to see how others react. Some create fake online profiles just to try out different sides of their personality. The problem with such experimentation is that it is conditioned almost entirely by online reactions, often reactions of strangers. There is little room to form thoughts independently.

Throughout the book, Turkle brings up the question of solitude.
